Founded in 2003, HOPE Community Medicine is a nonprofit, Federally Qualified Health Center (FQHC) dedicated to delivering high-quality, affordable healthcare to the medically underserved communities of Deep East Texas. With a mission to ensure no one is denied care due to their ability to pay, HOPE provides comprehensive medical, behavioral health, OBGYN, and pharmacy services to individuals and families across all backgrounds.
What began as a small clinic has grown into a multi-site healthcare organization grounded in compassion, faith, and community. Our team is committed to reducing healthcare barriers, improving patient outcomes, and expanding access through innovation, outreach, and patient-centered care.

The mission of Health Opportunities for the People of East Texas, Inc. is to provide families in the medically underserved counties of deep east Texas with accessibility to high quality healthcare regardless of their ability to pay, being underinsured or any other barrier that would prevent them from getting the care they need.

Our purpose at every Hope Community Medicine location is to provide each patient with quality healthcare. By taking a more personal approach to care, we ensure each patient is treated empathetically, taking into consideration the patient and not the ailment.
This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n) and an FTCA deemed facility.
This health center receives HHS funding and has Federal Public Health Services (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
